Abstract

Objective: Analysis the changes in trend of ampicillin/sulbactam utilization after introduction of “guideline for high dose ampicillin/sulbactam use in treatment of infection caused by carbapenem-resistant Acinetobacter baumannii” at Bach Mai Hospital.

Subjects and methods: The study included drug utilization data from Bach Mai hospital for the period 2021-2025, measured as defined daily dose per 100 bed-days. An interrupted time series analysis was conducted using an ARIMA intervention model to evaluate the changes.

Results: Following implementation of the guideline, the trend in ampicillin/sulbactam utilization increased gradually at a rate of 0.165 DDD/100 bed-days per month (p < 0.05). The Intensive Care and Internal Medicine divisions showed significant changes in utilization trends from the intervention point, with increases of +1.595 and +0.074 DDD/100 bed-days per month respectively (p < 0.05). Across clinical departments, ampicillin/sulbactam utilization varied substantially, with the most pronounced changes observed in units managing patients with severe infections.

Conclusion: Study showed changes in ampicillin/sulbactam utilization after introducing guideline and applying apparently activities in antimicrobial stewardship program. heterogenous changes across departments suggested the difference in antibiotic utilization patterns. Which hightlighted the importance of periodically monitoring, review to ifentify and inform targeted acitivities in antimicrobial stewardship program.
