Abstract

CKD-EPI FORMULAS WITH TC-99M-DTPA RENAL SCINTIGRAPHY IN KIDNEY DONORS

Objectives: (1) Assessment of Glomerular Filtration Rate (GFR) using MDRD, CKD-EPI Formulas, and Tc-99m-DPTA Renal Scintigraphy in Kidney Donors; (2) To compare GFR values obtained from the MDRD and CKD-EPI formulas with those from Tc-99m-DTPA renal scintigraphy in the studied patient group.

Participants and

Methods: This cross-sectional, retrospective descriptive study involved 359 adult voluntary kidney donors at the Nephrology-Urology and Hemodialysis Center of Bach Mai hospital from January 2016 to December 2022.

Results: The mean eGFR according to the MDRD formula was 100.99±21.61(ml/min), according to CKD-EPI: 105.53±14.75(ml/min), and according to renal scintigraphy: 115.89 ± 14.72(ml/min). The scintigraphic eGFR showed a moderate correlation with the eGFR calculated by the CKD-EPI formula (r=0.405, p<0.001), whereas the correlation with the MDRD formula was not significant. Both MDRD and CKD-EPI formulas underestimated eGFR compared to the scintigraphic eGFR, with statistically significant differences (p < 0.05). The bias, precision, and accuracy indices all showed that the eGFR calculated by the CKD-EPI formula had higher performance than the MDRD formula when compared to scintigraphic eGFR (P30 were 87.56% and 94.5%, respectively), with statistically significant differences (p< 0.05).

Conclusions: The CKD-EPI formula exhibited a better correlation with scintigraphic eGFR than the MDRD formula. The bias, precision, and accuracy indices demonstrated that the eGFR calculated by the CKD-EPI formula had higher performance than the MDRD formula when compared to scintigraphic eGFR (P30 were 87.56% and 94.5%, respectively) in adult kidney donors with normal renal function. This provides clinical physicians with a more accurate, simple, and convenient tool for assessing renal function and aids in selecting the most suitable kidney donors.
